HNHS fall sports physicals set for Tuesday, June 2, at North Arena

Huntington County students who plan to participate in high school and middle school sports programs this fall can receive a sports physical Tuesday, June 2, at 5:30 p.m. at North Arena.

The physicals are for athletes who are not currently under a doctor's care for an injury or medical condition. Anyone with a chronic condition that requires constant monitoring by a doctor, such as diabetes or blood pressure, must have a physical form filled out by their own doctor.

In addition to physicals, students may also receive tetanus boosters, meningitis vaccine and hepatitis B vaccine from the Huntington County Health Department during the June 2 session. Childhood vaccination records will be required for the completion of the hepatitis B series.

The vaccinations will be offered in Room 708 of Huntington North High School, just inside Door 28 (the main fieldhouse entrance), from 4:30 p.m. to 7:30 p.m.

Both the physical form and the vaccination form must be filled out and signed by the athlete and the athlete's parents prior to the physical or vaccinations. Forms are available outside the Huntington North athletic office.

Physical forms can also be printed off the athletic department Web site, www.hccsc. k12.in.us/HNAthletics/index.html.

Payment for the physicals and vaccinations can be made with cash or checks.
