Jack T. Horn

Jack Thomas Horn, 78, of Columbia City, IN, and having family in Huntington, IN, died Wednesday, Aug. 1, 2018, at 4:14 p.m. at Parkview Regional Medical Center in Fort Wayne.

Mr. Horn’s formative years were spent in Pierceton. He graduated from Pierceton High School in 1958. Following his graduation, he enlisted in the United States Navy. He served aboard the USS Yosemite, and received his honorable discharge Aug. 12, 1964. He and his wife have lived in Columbia City since 1970. For 30 years, he was employed by Dana Corporation, Fort Wayne, in the assembly plant.

He was born May 14, 1940, in DeKalb County to Ray and Wacilo Samuels. He married Patricia Ann Duff on Dec. 17, 1964, and she survives.

Additional survivors are a daughter, Vanessa K. (Brian) Bills, of Columbia City; a son, Troy T. (Carla) Horn, of Columbia City; a sister, Patsy (Don) Arms, of Huntington; and five grandchildren.

A service of celebration will be held at a later date.

Arrangements are entrusted to Smith and Sons Funeral Home, Columbia City.

Preferred memorials are to Churubusco High School football.
